FASEB Excellence in Science Nomination Submission Site

The 2010 Excellence in Science Award Nomination Submission process is comprised of six steps listed below. Print this page to help guide you through the nomination process.

Submissions must contain all minimum requirements. Once your nomination is submitted, you will receive an email confirming receipt of your submission. The email will also contain an Upload Approval Code, which will enable you to return to your Nomination Form and upload additional documents, if you choose to do so. All previously loaded documents will be saved.

Please do not use the "Back" and "Forward" buttons on your browser while completing the Nomination Form. Using these buttons to move between pages on the Nomination Form can hinder our ability to properly capture and process your submission.

STEP 1) Log In - Nominator Email Address & Nominee Email Address

At the bottom of this page you will find a Login Button. This is where you will access your electronic Nomination Application Form. Below are instructions to guide you through the application. Your submission must contain all minimum requirements. Once your nomination (with the minimum required documents) is submitted, you will receive an email confirming the receipt of your submission. The email will also contain an Upload Approval Code, which will enable you to return to your Nomination Form and upload additional documents, if you choose to do so. All previously loaded information will be saved.

STEP 2) Enter your Nominator Identification Information

This page gives you the opportunity to provide your Identification Information including Full Name, Society, Title, Institution, Address, and Phone Number. In order to continue to the next page you must provide complete information.

STEP 3) Enter Nominee Identification Information

This page gives you the opportunity to provide your Nominee’s Identification Information including Full Name, Society, Title, Institution, Address, and Phone Number. In order to continue to the next page you must provide complete information.

STEP 4) Upload Documents (Individual PDF's ONLY)

This page gives you the opportunity to provide the required documents listed below:

1 Nomination Letter

1 (C.V. must document all publications, leadership roles, mentorship, teaching, honors, and awards)

5 Reprints - Exactly

3 Letters of Support (Peers) - Minimum, (5 Letters - Maximum)

3 Trainee Recommendations - Minimum, (3 Letters - Maximum)

STEP 5) Review and make any necessary change to submission

Before submitting, you will have an opportunity to review your information for accuracy and to make corrections and/or additions. Updates can be made anytime up to the February 23, 2009 deadline.

STEP 6) Approve and submit Nomination for review...

Submitting your information is safe. Our secure server software (SSL) is the industry standard and among the best available today for secure information transactions. It encrypts personal information, such as name, address, phone, etc., so that data cannot be read as the information travels over the Internet.

The Deadline for submission is February 23, 2009.

Announcement of the 2010 Award Recipient will be posted on this website in June 2010. Nominators will receive notification prior to the website posting.

By submitting this data form and all accompanying documents (“the Submissions”), the Nominator agrees as follows: The Submissions become the property of FASEB and may be utilized by FASEB in its discretion, including by distributing the Submissions to members of the FASEB Excellence In Science Award Committee or other parties and publishing the Submissions on any FASEB website. FASEB reserves the right to select appropriate material consistent with FASEB style, standards, and limitations of space. In the event of errors in publication, the sole responsibility of FASEB will be to correct such errors in succeeding publication. Such correction is in lieu of any other remedy. FASEB expressly disclaims all other liability for loss or incidental or consequential damage whether arising from negligence, contract, or other cause to any party for errors in publishing. The Nominator shall obtain consent of the Nominee to the use of the Submissions by FASEB, and to the limitations of liability, as outlined above, prior to providing the Submissions to FASEB.
